- E-mail address
- Most recent statement for the assets or cash you want to transfer
- Credit Card or Bank Account Number to pay fees
- Details on the alternative investment you will custody at Millennium Trust
Choose Your Account Type
You have two options to complete the account opening paperwork. Both options require that you sign the documents and mail the original paperwork to Millennium.

To open a Solo 401(k), you need to call 800‐258‐7878 to speak with a Client Service Representative during our normal business hours.
